In a world where healthcare is becoming increasingly transactional, one practice in Wilmington, North Carolina is carving out a different path—one rooted in purpose, precision, and patient-first values. Founded in 2019, Tinsley Surgical, PA has emerged as a beacon of independent medical excellence, specializing in vein, vascular, and general surgery while transforming how surgical care is experienced by patients.
At the heart of this transformation is Dr. Ellis A. Tinsley Jr., MD, FACS, FSVS, a seasoned vascular and general surgeon whose mission transcends clinical metrics. “I see my work as a deep calling. I call it selfless exceptionalism,” he explains. “There is nothing more important in the world than working with people in their most vulnerable circumstance.”
From its inception, Tinsley Surgical was designed to deliver care in a way that prioritizes the human connection. The practice has grown rapidly since its founding just six years ago, and today it boasts a multidisciplinary team: one full-time vascular/general surgeon, one fractional vascular surgeon (with another full-time surgeon joining soon), two physician assistants, two nurse practitioners, and a diagnostic ultrasound lab with six technologists. Despite this growth, over 90% of new patients are still seen by Dr. Tinsley himself—a testament to the practice’s deeply personal approach.
The foundation of Tinsley Surgical is built on four core values: being patient-centered, accessible, efficient, and effective. Whether a patient is seeking help for varicose veins, peripheral artery disease, or general surgical concerns like hernias or abdominal pain, they are met with a philosophy of care that seeks to understand and address their goals from day one. “We start with a clear assessment of the patient’s goals,” says Dr. Tinsley.
“Our philosophy is to find a way to say yes,” says Mary Floyd, RN, MSN, BSW, DNPS. “This encompasses our work whether it is scheduling an appointment, accepting a walk in, responding to an urgent need or handling the myriad of social and ancillary needs that patients have daily.” Mrs. Floyd has 28 years of nursing experience and soon will graduate with a Doctor of Nursing Practice from University of North Carolina at Wilmington. She is intimately involved in every detail of the practice but specializes in how patients are treated and their overall experience.
Dr. Tinsley is no stranger to the complex evolution of the American healthcare system. A second-generation independent practice surgeon with over three decades of experience, he has watched hospitals grow into sprawling systems where bureaucracy often undermines bedside care.
“As systems for delivery and payment become larger and more business-oriented, the actual care at the bedside is challenged,” he notes. “People are not just boxes to be moved around in a warehouse. Each one is different.”
For Tinsley, staying independent isn’t just a business decision—it’s a moral imperative. He values the ability to rapidly implement new technology, control operational efficiency, and foster a culture that supports both staff and patients. It’s also allowed the practice to remain nimble and adaptive, even in the face of mounting economic and systemic pressure.
But running an independent practice comes with challenges. “You really have two full-time jobs,” he admits. “You’re managing a small business with HR, finance, real estate, and equipment—while also practicing medicine at the highest level.” The key, he says, is building a great team and learning to speak the language of both medicine and business.
What sets Tinsley Surgical apart is not just its ethos, but its scope of services. The practice offers a full suite of vein treatments, including radiofrequency ablation, sclerotherapy, and chemical ablation. Its vascular care includes managing conditions like peripheral artery disease, aneurysms, and carotid artery disease, with both in-office and hospital-based procedures. The general surgery division handles everything from laparoscopic hernia repairs to thyroid surgeries and skin cancer excisions.
“Our patients enjoy a variety of diagnostic evaluations on the same day as their visit,” says Dr. Tinsley. “We’ve also invested in in-office procedures to reduce wait times and costs.”
This integrated care model means patients can access diagnosis, treatment, and follow-up—all under one roof. It’s a model that is not only clinically efficient but emotionally reassuring for patients navigating complex conditions.
Beyond treating patients, Tinsley Surgical is deeply committed to shaping the future of medicine. The practice runs a pre-professional program for post-graduate college students aspiring to careers in healthcare. Its alumni have gone on to study at prestigious institutions like the UNC School of Medicine and Duke’s Physician Assistant School.
Tinsley Surgical also partners with multiple academic programs, hosting student rotations from UNC, ECU, UNCW, CFCC, and several physician assistant schools. It’s a purposeful move that ensures the next generation of medical professionals understands not just clinical protocols, but the human spirit of care.
“Teaching others who are younger and just starting out is a critical part of our mission,” says Dr. Tinsley. “We’ve been given gifts and opportunities—and it’s our responsibility to pass that on.”
What unites the diverse team at Tinsley Surgical is more than just medical credentials. From nurse practitioners to technologists, every team member is chosen not just for skill, but for alignment with the clinic’s values: honesty, energy, empathy, and a relentless pursuit of excellence.
“We’re not perfect, but we’re working hard to be excellent,” the practice proudly states in its internal vision. And that vision resonates with patients. With options for same-day and telehealth appointments, and a care model that emphasizes listening before acting, patients feel seen—not just treated.
Looking to the future, Tinsley Surgical shows no signs of slowing down. With plans to bring on additional surgical talent and continued investment in diagnostic and treatment technologies, the practice is positioned to serve an even broader segment of the community while holding fast to its founding ideals.
When asked how he has remained committed to the independent path despite industry consolidation, Dr. Tinsley reflects on the people who have supported him, the faith that guides him, and the sense of purpose that fuels his work.
“I was born into a great supportive family. I’ve received certain skills and traits from my Creator. That is my privilege,” he shares. “My responsibility is to use my gifts to improve the world one patient at a time. This involves treatment at the bedside and advocacy for access to quality healthcare in the community. To Him be the glory forever.”
In an industry often overwhelmed by complexity and cost, Tinsley Surgical stands out for its clarity of purpose and the simple yet profound idea that healthcare should feel like healing—not a transaction. One patient at a time, this small independent clinic is making a big difference.
It’s also setting an example for the industry. Tinsley Surgical proves that when values lead, outcomes follow. With the courage to remain independent, the curiosity to keep innovating, and the compassion to always put patients first, this practice isn’t just succeeding—it’s leading.
